Thank you David. I replied again and don't know if you saw that message. It may not have been approved but I have had further diagnosis since the claim was joined in 2019 around beginning to end of 2023 and I didn't inform the DWP. I don't even know if I was supposed to because partner was lead claimant and the one being assessed. Just worried because of my brain fog, fatigue and stupidity etc I've messed things up. Kept making mistakes on the UC migration form. And when it asks if we care for someone I clicked yes because we're both sick and our only support and I don't know if I should have or not. I had to keep making changes and my partner submitted at one point and I hadn't and realised I needed to make a change still. Just been a nightmare because we've had no help to do this part which I really needed. I did call Citizens advice but it just made me more confused and I just would end up crying on the phone. So I do really appreciate the solid and simple advice I get here. It makes a huge difference. When I was looking for the letter for the ESA I noticed I'd been sent a P45 when our claim was joined. Is that usual. I think I didn't understand at the time or question it because the payment we were getting was saying for a couple. I hate that I struggle to understand all this so much. I feel really silly for taking up so much of people's times. Again though I really appreciate what you've said and I will send the message.

Yes I have replied on your other post.
I am sure it would have been a P60 which is because your partner received a taxable Contributions based Benefit.

Hi, I actually received a P45 that had a leaving date of 21/05/19 I think I didn't know what it was and still really don't and just put it aside at the time. Do I need to worry about it?

Hi David, thank you again. I sent the message. Still unsure of whether I need to self certify meanwhile. Scared of the claim being ended. I submitted on 13/2 and ID verified 14/2 . Just to make sure I've given you the correct information. This is what was written in my journal about a fit note:
When you applied for Universal Credit we told you that you did not need to provide a fit note.
We have since found that you have not yet had a Work Capability Assessment outcome for your health condition.
A Work Capability Assessment is something we refer you for when you have an ongoing health condition that restricts your ability to work.
What this means
You must now get a fit note from your GP or healthcare professional, if you still have a health condition or disability which restricts your ability to work.
You are allowed to self-certify for the first 7 days when you tell us you are not fit for work.
The start date of your fit note should be the date you made your Universal Credit claim plus 7 days. For example, 14 March + 7 days would be a fit note start date of 21 March.
Report a fit note in your Universal Credit account.
What happens next
If you do not provide a fit note, or a good reason for failing to do so, we will consider you to be capable for work.
This means you may be asked to look for, and be available for, work and work-related activity.
And there a commitments review booked to agree to new commitments and review my situation at the job centre.
And lastly update your work profile before your appointment

Hi Blissfulblues,
A P45, is just a document from HMRC that shows what tax and other contributions you have paid. You find that a new employer will ask for this, so they can pay you correctly, based on your past details.

Hi Blissful,
I had this happen to me. You need to visit your GP and get a fitnote. After 3 months I believe it is, the DWP will get you to fill out a UC50 form, and then you'll be assessed to see whether you're placed in the LCW or LCWRA group, or remain in the looking for work group.
