Been Refused DLA & ESA Stopped

Have a look at page 60 of the The Best Possible Disability Living Allowance Claims For Adults On Physical Health Grounds guide, this has a checklist and a report card for a visit to the doctor.
We don't have a similar one for ESA, but if you print our the WCA criteria from Understanding ESA guide you could take it instead.
Sit down with your GP and go through both, so that he properly understands how you are being assessed and ask him to write two new reports, one for DLA and the other for ESA, based on these assessment criteria.

Just a thought, but why not contact your employer, and ask them for a report of why you were unable to work. Was it under their guidance that you had to leave, or did you leave of your own accord. A further thought occured too - ask your doctor to see a specialist, there must be an underlying reason for this migraine.

Hopefully now i managed to open (with a breakdown of tears) he will be more understanding.
Jaxie.... Fortunately when i was dismissed on grounds of ill health, the matron that headed the final sickness review is a lovely lady. In her report of dismissal she did state that not only am i having chronic migraines i also am suffering serverally with depression. I do have a copy of that but didn't submit it when i applied for either DLA or ESA. Do you think i should submit it for the appeal hearing?

It's possible he didn't write it down in order to be helpful. If in the future you apply for insurance or apply for a job, you may find that you are asked to sign consent for all your medical records to be released. Not everyone is comfortable with information about suicidal ideation and self harm to be included in reports that third parties may read.

I was just about to post the same not putting it does you a favour regards life insurance the GP makes a call on what ever you say and how to write it down, unless its fact
If anyone did top themselves after saying they might to gp as the person is not there to say other the gp is safe in saying the person was upset but not that upset

I certainly would submit it. It is all extra evidence for your claim. I know that telling strangers about your health problems is so very difficult, but it is also essential so the DWP can understand exactly what problems you are facing in your life.
Hang on in there sweetie. Dont forget to contact the DWP soon, to make sure they have the other report from the doctor, and the one from your work. Although, if you only have one work report I would copy it again first.
