We understand that rollout of the new Disability Living Allowance and Attendance Allowance medical report forms used by visiting doctors is now complete. We have updated
The Best Possible Disability Living Allowance Claims on Physical Health grounds
The Best Possible Disability Living Allowance Claims on Mental Health grounds
to reflect changes in the medical report forms. We'll be updating the children's DLA guides and the AA guide within the next week.
The new report forms no longer require the doctor to take a statement from the claimant, which is instead replaced by a detailed 'Typical day' section.
You can download a blank copy of the new Disability Living Allowance and Attendance Allowance medical report forms from here.
Our guides now include details of the new medical report and a warning about the increasingly common practice of EMP's prying into every room in a claimant's home and how to deal with it. The Medical Visit Record Sheet has been updated to deal with the medicals and the prying regime.
