Form used by DLA to request info from doctor

Is there a form which the DWP uses when it writes to a doctor to get information about a patient who's claiming Disability Living Allowance?

If so then is there a link because I can't find anything in the member's area here.

(There is a form called ESA113 for info about ESA claimants but the one I am looking for is the equivalent for DLA claims.)
