DLA change of circumstances

sent off my letter asking for review of care component last week with my letter i sent consultants and doctors leters plus 2 other support letters and over 50 enclosures about my health difficulties.
just got a phone call saying they are sending me a claim form and they will look at my mobility component as well.
I even checked with Citizens Advice who advised me that the action required was just a letter.
where do I go from here
