If your NI number is not on a page, that of itself wouldn't be sufficient grounds for challenging it, but it leaves it open for you to argue that that page is from another report that's been mixed up with yours if the contents of that page justify it.
For more detailed guidance on challenging reports, see our guides in the Help for Claimants section.

It's just not possible to advise you in any detailed way without knowing all the facts of your case, so we can only give general advice.
Assuming you are talking about your GL24, then you don't need to go into great detail, but if you can be specific about things, it will help the decision maker to revise their original decision.
I really do think you need to look at the guides in more detail. I know that that doesn't sound helpful, but like I say, we can't advise in any detail on an individual case.

Hi Jacqui. I don't know much about DLA, but wonder if a DLA expert at the CAB could help you with the letter you want to write?
It is clear that you have a high level of difficulties and care needs and am very sorry you are struggling with the additional task of writing to the DWP requesting they reconsider their decision.
Was your original claim very detailed? If so, you may not need to write very much. I would have thought letters from friends and any other evidence illustrating your support needs should help.
I really wish you well with it all.
Best wishes
