New ESA50 10/10

We've recieved the new ESA50 Limited capability for work questionaire today.

I've searched the site for a guide but can't find one that is for this new form, is there a link someone can send to get a guide to help us fill this in please?

dont think there is a guide for new form

any chance of typing in all the questions ?

The layout looks new, it takes into account fluctuating conditions somewhat in that there's no a 'it varies' tick box,

we've not seen this form before, it's pink! last time it was gray?

Question 1 isn't until your 8 pages into the form, there's a whole heap of questions before that.

Everythings got 'fuctions' after it at the top of the pages

Under walking it asks if you normally use walking stick or crutches.

If it is the new one I'd be happy to scan and email to you later tonight
