IB50 Form

The letter and form came from Atos, I am unsure who I sent the letter to once I have clarified no IB113 form has been issued, ATOS, Jobcetnre or both? To confirm if they have or have not sent the IB113 do I ring the number on ATOS form which is an 0800 or do I call the jobcentre dealing with my claim (does it matter?)
Does this form go to my GP or hospital consultant as I vaguely remember putting down my consultants name on the form but have since changed hospitals but with the same GP. and should I put my GPs name and surgery details down for good measure. I have already informed my GP to expect the form.
Is it likely or usual that I will have to fill in the 1b50. I have had it for 2 weeks already and feel a bit panicked about it now. When I speak to the job centre can they tell me i dont need to fill it it or do i need it in writing.

"You can do this either by contacting your GP’s surgery or by contacting the DWP.
We would suggest you try both."
So I would ring your GP and the JCP dealing with your IB claim. Check with your GP first as you're more likely to get a quicker answer from them then if they haven't received one ring the JCP office.

The procedure for the issue of form IB113 has changed since Atos started issuing form IB50.
Previously the IB113 was issued to your GP prior to completion of the IB50, but now it is issued only if necessary on return of the IB50.
If the Jobcentreplus office is telling you not to complete the IB50, you must get this confirmed in writing in case there is any dispute at a later date. You should also write to Atos about your potential exemption from the PCA, in accordance with the exemption criteria as defined in the section in the "spotlights" area at the top of the forum.
It is advisable to send any correspondence to Atos and/or the DWP by recorded delivery.

276. If the customer was found to be exempt under the Personal Capability Assessment at the last referral, clerically issue form IB113 at the re-referral stage to consider the customer’s potential exemption,
before issuing form IB50."
As you see this says that an IB113 should be issued before issuing an IB50. So is this now not the case?
So if I it correctly the situation now is that everyone irrespective of whether they were exempt at a previous claim is sent an IB50 when their case is up for review, then depending on what ATOS conclude about your IB50 an IB113 is issued (either by ATOS? or the DM?)then on receipt of the IB113 ATOS advise the DM accordingly as to whether exemption continues.
Is that now the case?

CD the Short Term Benefits Guide Pt 28 in the members area shows the following:-
"Issue of IB113 in re-referral cases.
