Thank you again for the advice, I hope it will benefit other readers as well. I've learnt a lot from reading posts on this forum.
The ESA3 was promised within 48hrs on Friday at 5.30pm so will have to be here in the morn as there's no post on Sunday!
The ESA85A was promised within 3 weeks so I will send an SAR if it doesn't come within a month.
Yes, I already knew what the LT54 is, but he totally blathered me by saying it was a Living Together form.

The ESA 3 is not available in a PDf version, so you will have to wait for the hard copy to arrive.
The one that I provided the link for is purely a sample provided as the result of an FOI request on the what do they know website.

Just an update which may benefit other users.
The ESA3 I requested on 11th Oct still hasn't turned up so I rang again today 22nd (got straight through no holding for 30mins as before.) Asked again if I should have ESA IR or Pension Guarantee Credit and he said it all depends, fill the form in then when get results, decide what the best option would be. A right gobledegook answer!!!
Went online on the Pension Credit Calculator to see what that came up with... It said I should get Income Support.. . What? Totally wrong so don't ever trust that! My IS stopped when I got PGC, surely they should know. I certainly left feedback.
So rung Pension Credit and she said it depends on my Pension Forecast. If I have 30 yrs contributions then PGC is best, if not then ESA IR is best. (I think it was that way round)
So rung Pension Forecast, she wouldn't tell me over the phone as it was being calculated, will get in post in about 10 days.
My ESA CB starts on 29th Oct so hope I don't miss out on anything while I decide what to claim.
