There is detailed advice on how to write a GL24 in the ESA appeal guide in the members' area.

You need to return the GL24 within four weeks of the date of the Decision that you were Fit for Work.
Have a look at our ESA Appeals guide, which explains how to complete the GL24 form.
You don't need to send evidence with the GL24, but if you are able to compile an evidence pack in the next few weeks, you should send it to the DWP as it can be included in the Reconsideratio that the DWP are obliged to carry out as part of an appeal. If sufficiently compelling, it can result in the Decision being overturned by the DWP, without the need for an appeal.

I feel sorry for you. I was in the same position as you last week. If you haven't got a copy of your medical report, ask for a copy. This isn't the scoring sheet and decision, it is what was actually written about you and the questions you answered in your medical. I completed my appeal form with an attachment, breaking down every point I disagreed with or things that had been presumed by the examiner or things that were written that were untrue. I found that was the best way to approach it and felt a sense of relief once it had been posted off. I am still gathering info from my GP, MP and written statements from my family, as they know how my illness has affected me, and I will forward these on as soon as I get them.
Good luck with your appeal. x
