Help! I didn't keep a copy of my filled in ESA50

Does anyone know if I can get a copy of my ESA50 form once I've filled it in? I stupidly forgot to make a copy (very unlike me, I copy EVERYTHING) I have my health assessment coming up and I want to go over what I wrote just so I'm on top of things. I've been reading how difficult the assessments are and have scared myself to death! I really hope someone can help. Thanks

You can either telephone or if you prefer, write to Atos and/or the JCP/BD office which deals with your claim, and ask them to provide you with a copy of the completed ESA50.

Is that the same for DLA crazydiamond?

Yes it is, but you will have to contact the DLA Unit at Blackpool for a copy of your DLA claim.
