Medical Report Form was created by mumsgirl
I had a medical last Oct and i expect to be going for a reassessment soon because I've received another form! When I got the last medical I asked for my Medical Report Form. I passed the test, getting 15 points for 2 questions. But for all the questions I failed it just says 'none of the above apply'. Why do they not give you a report of the questions they asked you and your answers, and how could you appeal if you don't know what you're appealling against because it says 'none of the above apply' I hope you understand what I mean!

You can request your ESA85 Medical Report which contains a full explanation of all the points raised at the ESA medical, just give your local DWP/JCP office a call.

Are you referring to the ESA85 when you mention 'none of the above apply'?
If so the reasons why the disability analyst thinks that none of the point scoring descriptors apply will be below, in the section named: Prominent Features of Functional Ability Relevant to Daily Living
You can always challenge what the disability analyst says on your medical report if you disagree with it. The DWP will also send their full reasons for disallowing your benefit with the appeal papers, which again, is open to challenge.
