my mums renewal for dla is up very soon. if the dwp send out a short form do i fill it in and pray or should i ring up and ask for the long form? so i can put everything on as things have changed in the last 2 years.
she now has distolic heart faliure but is not symotomatic enough to see the heart failure team :angry: even though to me she is. they do not hear her when she moves. plus she has more health problems than that. thanks

You can download the longer DLA form here:

www.dwp.gov.uk/advisers/claimforms/dla1a_adult_print.pdf

You can save it and add or amend your responses on the form. Then send it in lieu of the shorter form (I would enclose a covering letter).

Or use the downloadable form above as a template to formulate your responses and send in the shorter form with your responses attached on continuation sheets (remember to put name and NI number on every sheet and number them '1 of x', '2 of x' etc).

Personally I would use the downloaded longer form but either way it is important all information is sent so don't be afraid to use continuation sheets even with the long form if your responses do not fit into the boxes on the forms.

Don't forget that although your mum has a new diagnosis, this is only of secondary importance, it is her actual care/mobility needs that DLA awards are based on.
