GP wont fill in ESA50

I had written a list of my diagnosises and how it affects me for her as I am new to the area but it seems I have wasted my time as she refused to look at it, not how they do it in Sweden!
I have Fibro /CFS and Joint Hypermobility amongst other things. Problem is I havent seen a rheumy here, only a medical doc who saw me for about 5 minutes and gave the usual diagnosis "just your fibro getting worse, try this tablet" and cant think she knows much about my condition or how it affects my daily living. Also the hospital is about 2 hours away as have moved house and dont want to drive that far trying to speak to her, dont even know if that is possible.
It takes ages here to even get a GP appointment, once I waited 8 weeks! As my form has to be back by 20th June I am starting to panic!
Any ideas?

Its page 3 to be completed by doctor, includes diagnosis, other medical condition and how those affect me.
Its maybe a different form because I live abroad, ESA N 50
worried it wont look good if I dont have any info from GP

I asked someone whos a brit out there married to a local nurse .. he says get your dr to send you to the right type of consultant and for about 300 kroner they will do the report take along all the reports you have ask for one that speaks English ... which is easy
i have no idea how much that is in UK pounds
I know the pay rate for consultant over there is half UK rate

No its not the details you quoted, they are on page 2 though worded slightly differently.
This is Part 3 on the last page (28)
and says :Statement by the customers GP or hospital doctor : this statement should be completed in light of your knowledge and records of your patient"
It is the old form I have been sent.
