my ZERO points medical - finally got the report !

The form clearly states that the ATOS rep was with me for 40 mins but that the report was completed 4hrs 20 mins later.
In the time between visiting me and the completion of the report a huge amount of information has been recorded and as far as I am concerned a large amount of it bears no resemblance to what was asked or replies I gave.
There are a large number of clear contradictions including one which states that I only leave the house with the assistance of my carer only to be followed later by can leave the house quite freely without assistance.
There is also a claim that there is a certain activity that I have carried out, and continue to carry out, despite me never being asked if I actually carry out that activity at all.
I have read the report 3 times now and I am still stunned at how many inaccuracies are contained within it.
Naturally I will rip this report to shreds line by line and add it to the appeal I have already submitted based on the telephone call advising me I had scored 0 points.

Sorry to hear about your inaccurate medical report, though I suppose you didn't have high expectations in that area!
Medical reports are usually completed a matter of minutes after a claimant leaves the examination room, but I would imagine for home medicals the doctor would have to complete it back at the office, perhaps after carrying out other medicals.
One word of advice. Although it's tempting to rip a medical report to shreds line by line, especially when you are furious about incorrect statements about your health that are sometimes hurtful; it's best to concentrate on the descriptors that you feel apply, and the inaccurate statements that denied you the points. As well as any overall contradictions and inconsistencies.
An appeal submission pointing out everything that was wrong with a medical and consequential report could potentially run into a hundred pages with the danger of the key points being overlooked.

Agree what you need to do if write a statement correcting all the points backing it up with referance to medical reports to back up your verson of the facts ... so for example you could say, that atos dr says I can go out on my own... you can say "I am unable to leave house without support of my full time carer" and add ref to details of carer hours worked and any other paperwork or reports you have

my case is already at reconsideration and my MP is involved but I will still draft a reply to the medical and send it off as it may be useful.

yes your "rage" "malpractice" means a drs operated on you and botched it ...

I had exactly the same experience as you. My ESA85 was FULL or errors ... misinterpretations, downright untruths, and omissions of important evidence I'd given. It was completed during (and within 20 mins of the end) of my actual interview, so time isn't necessarily the culprit! From what I've seen in this forum such travesties are, sadly, par for the course.
Initially I vented my anger and disbelief by writing a line by line rebuttal. That helped 'get it off my chest'. I then cooled down, went back and picked out the important bits:
(a) those items that would affect points allocation - I explained the true facts in each case
(b) at the start, just to establish the context, I included a couple of particularly awful inaccuracies/contradictions which wouldn't affect points but demonstrated the 'poor quality of the information in the report'. I explained that's why I'd included them. It's great if they've made a real blooper, because it shows just what rubbish ATOS churn out. For example, my report said that I had no trouble hearing when called in the waiting room. Which is clearly a standard computer-generated tick-the-box item to deny points in the hearing category. No biggie as my hearing is fine, BUT elsewhere the report stated (correctly) that the nurse had to fetch me from the car (outside the front door) where I was lying down (to conserve strength). It also said I had 'moderately high sitting blood pressure' when it was 180/90 which ain't moderate as any doctor will tell you! Nice to include a significant medical inaccuracy!
I had a couple of friends read my appeal statement and give feedback - just to make sure that Mr Angry hadn't managed to creep in again and dilute the focus.
