Have a look at our DLA guides as they will explain the process, how to explain your disabilities and what evidence will help your claim.
When you are ready to do it, give the DLA office a call and they will issue you the paperwork.

Hi,
DLA is not based on diagnosis but on your care and/or mobility needs as a result of your disability. Any diagnosis is only really of secondary inportance to your needs.
As obviously we cannot see your or chat with you, it may be wise to get some face to face advice from a Welfare Rights/CAB/DIAL etc and go through your needs, primarily to ensure your current award is 'safe' as that could be reduced, and also to assess your chances of receiving an increased award.

edit - sorry Gordon your reply wasn't there when I started to type.
