Completed App ESA50 Questionnaire - Special. Ltr was created by cmarie510
Hello Everyone,
With the help of your guides I have now completed the latest ESA50 Questionnaire. To ensure I completed accurately and well I completed it in sections. Doing it this way, starting immediately and always referring to your guides along the way made this a less daunting task. So, thank you.
Yesterday I went to my GP, updated her and asked if I might get copies of my specialist letters. She was fine with that but said I should not bother as they would not be looked at.
Just this morning I rechecked the ESA50 form. There is a box to tick if you are including any medical notes/records. I do have my last letter from the neurologist and thought I might include a COPY of that as well as a COPY of a letter from my spine surgeon.
I feel that as I am including copies only what is the harm and as stated they have a box on this latest 50 form which indicates that you may send in supportive documents.
Please, would you advise me regarding this matter.
Many thanks,
cmarie

Nothing on this board constitutes legal advice - always consult a professional about specific problems

hi Seabury,
I certainly would include copies of your specialists letters. It may be that they will not be looked at, but if the Atos health professional clearly ignores the evidence provided and you do not get an award of ESA, then you can include this as part of your grounds of appeal.
Good luck,
Steve
