Does a care plan hold much clout ?

Hi all well i posted already about being one of the many people who have had one of the horrendous frightening letters stating "they have info concerning my eligibility of my Dla claim " i have now gathered most of the info together and my local disability group helped me fill in the form, tbh i cant remember at all what i wrote on my form 2 yrs ago as i am on Fentanyl patches and messes with my head, but my last claim they just wrote to my doc and also i sent in a care plan, what i want to know is does a care plan hold much clout..I have a OT report but its from 18 months ago should i include that or not, and should i include copies of all my hospital appointments.
I have now been awarded 14 hrs of care to help me with my every day needs and of course i will be sendingthe care plan it with my form.
I am very worried and scared about the poss chance someone has made a malicious call about me but there is nothing i can do about it now but send in the form and wait and see the outcome

i two been wondering this, as i have one for daily caers comming in from the contracted company but trying to have correct details on what they help me with, and one ie, a cpa care plan, waiting to chnage soical wokers, over two thier breach of the soical workers code of conduct, how they drew it up, with out detailing uptodate medical reports and correct medical disbaibiltes and how in my medical rerods say, i long term , not able to work all , looks and not signed off, so not bale to use it, then still fromer privte medical reports also comfrims this, then gp in with making money with new dwp and governmnets , ie what can you do, and get his pay for filling out from, but aready failed to do medical referals for ones medical treatment apointments and one was 6mths delay, so no faith in ones gp,so ,to wondering when or if i use to date, ref to trying to get mamended care plan and cpa care plan, before now still trying
