Sorry forgot to say in my last post was that i cant remember what i wrote in my original dla form and i dont have a copy but my health has deteriorated and i have info to prove that will that go against me as i cant remember when certain probs happened as my memory is so bad because of the meds i am on and i am worried this will go against me, also having a care plan being awarded 14 hrs a week will that be helpful for my claim would it be as good as an ot report which i had last time, sorry if i have asked this in my last post but i have the memory of an 80 yr old lol

Hi all just a quick update finally sent in the dla80 form, have had to put it to the back of my mind as its making me sick with worry.My last claim i did not have any consultants reports but with my gp report and care plan they awarded me care and mobility, i am wondering if with all the changes they are being even more stringent.
Is the care plan an important document, my social worker in the adult disability plan is fab and they are welcome to contact her, i was awarded 14 hrs care by my local disabilities team so they must think i really need the help, but got a nasty feeling this time i am going to get turned down. I really need my high rate mobility as my car has packed in and i would like to get a mobility car, without a car i am a prisoner in my home.

Anything where an independant professional has reviewed your care needs and produced a positive report is evidence you should submit with your DLA forms.

Ty Gordon for your reply, it has put my mind at ease. Apart from my care plan and doc report that is really all i have so will just have to wait and see.

Try not to worry, I had my letter after completing my DLA80 (and having a medical), and nothing changed. I have kept my HRM and HRC indefinitely.
