DLA internal forms especially DBD385

Hi In the resources section, I've tried to link to the 'other DWP' resources and I can't find out how to get copies. So could someone let me know how I access them.
Particularly I am after the full 'Medical Services advise/evidence sheet - form no. DBD385.
I'm having to appeal DLA again and in the tribunal pack it refers to advice given in Section 6 - and guess what!!! I do not have a Section - included so I want to know what is included in this section so I can be informed in my request.

I think all the DWP DLA resources are listed on the main DLA page in the members area.

I certainly don't recall a DBD385 and I can't find one using our search facility.

But if this is refered to in your appeal papersthen ask the DWP to provide you with a copy. You could also contact the clerk to the tribunal and ask that a judge issue a direction for the form to be included in the bundle.
