You say that there is a gp that know's your husband best. Put his name down as well as the pain clinic and consultant. Explain in the additional information section that consultant and pain clinic are not usually willing to help and suggest they contact your husband's gp. Also would your husband gp be willing to put some information in with the dla form.

I attend a pain clinic and every time I attend an appointment I receive a letter that the consultant is writing to my doctor updating him on my case. From the sound of things your pain clinic isn't doing this and if they are they aren't including you in the loop?
These letters were invaluable to me when I was applying and re applying for my DLA. If the pain clinic are writing to your GP you can ask for copies of the letters and include them as evidence.
I'm not sure if I am right saying this but surely there must be some sort of correspondence between the Pain Clinic and your Doctor otherwise how can your doctor know what they are advising, treating, changing medications etc?

I get copies of letters from all the differnt drs I see at hospitals, this morning when I went to see my gp he had to fill in a form to send to fax another hospital so they can see me and on the blank form the give drs to fill in all the details was a box to ask at that early stage did I want all copies of letters copied to me .. and a new one to me was another box asking apart from my gp did I want anyone else to get copies too ? never come across that one before ???
