dla/incapacity

Hi All
Can anyone help me.
ive been on incapacity for ten years due to mental and physical health.
I also receive both high rate motobility and high rate care component for indefinate period.
what happens to me now?do i have to be reaccessed or go to medicals i'm very confused and scared about the future.I suffer from bi polar,personality disorder chronic anxiety and agorophobia.I have losr the use of my right hand and have only partial movement in right legthis eas dou to me contracting an mrsa virus during a stay in hospital
Please can somebody give me some advice
