my symptoms dont fit the forms

problems with speech - I have very dry mouth and unless i sip water or chew gum constantly my mouth dries up and I cannot speak or eat or sometimes even breathe properly.
these happen on a daily basis but maybe just for minutes at a time. this is the case with all my symptoms so the whole form just wears me out. i also suffer severe depression due mainly to the constant pain and frustration and I am uncertain which way to answer the questions sometimes.
so any advice on which box to tick please?

hi, this is my first post and to be honest I am really struggling to make sense of all the forms and the way the system works. I have autoimmune conditions ie... autoimmune hepatitis (with cirrhosis), sjorgrens syndrome, rheumatoid arthritis, carpel tunnel, each one causing a ruck of problems in day to day living. the problem is when i come to fill out the forms none of it fits? for instance on the incapacity benefit form : problems with eyesight - i cannot honestly tick any box due to the fact that my eye problems are not prevalent all the time. I get blurry vision, sometimes I cannot see for several minutes if the light changes

you could consider putting the word varies in the box and using the additional information area on each page to explain the range of frequency, variation or fluctuation.
