DLA Resources

Please can someone help. I can not find the following on DLA resources site, but it states of this site they are on here.

* Details of how to ensure you get proper notice of a medical visit, rather than a phone call the Disability Living allowance feedback:
* A copy of the medical report the doctor will complete;
* A list of the questions the doctor is likely to ask you; (THIS I DID FIND THANKS)
* A highly effective Medical Visit Record Sheet for you and your accompanying person to complete and use as evidence at a tribunal if necessary;
* Details of the ‘oral warning’ the doctor may issue you with if you take notes;
* Information about some doctors habit of ‘searching’ your home.

Also I have posted replies to my post similar to this and they have not appeared on the site? Odd.

All the information listed is in the downloadable guides to claiming DLA on physical and on mental health grounds in the members area. Check the chapter on medical visits in particular. The record sheet you will find at the end of the guides.

I'm afraid I don't know what might have hapened to your previous posts.
