What happens if a claimant is unable to travel to an ESA medical assessment due to the nature of the illness/disability? Would a 'Med 4' submitted with the claim be sufficient evidence of this fact?

In deed CAN a 'Med 4' even be submitted with the initial claim, or does it have to be a 'Med 3'? This is not made clear on the claim form.

If a claimant is unable to travel to an ESA medical assessment, documentary medical evidence will certainly be required from their GP.

Atos are very reluctant to arrange domiciliary (home) medical assessments for ESA purposes, so it is doubtful that a Med3/4, which as of yesterday are now obsolete, will suffice?

A telephone call to Atos customer service will confirm what medical evidence would be needed, in order to authorise a domiciliary medical assessment?
