PIP REVIEW capita have sent form to health professional.

Capita has sent forms to a psychologist and they have received them back from him. The form was actually addressed to a CPN who no longer works and whose name was on the original DLA to Pip form in 2017. They have also sent a form to the GP who received it and sent it back on 22.8.23.This form was addressed to the previous name of the surgery ( it's still the same address and the surgery have confirmed they received it and have returned it. Capita say they have not received it. I have rung them twice regarding this the person I spoke to said that the medical forms sent by them (capita) go back to the DWP first and are then sent on to them. I don't believe this. they seem to be using information that is from 2017 and I definitely put the new surgery name on the review form and the new phone number ( kept copies of the whole form). Yesterday 31/8/2023 a woman from Capita rang saying they are having trouble getting medical evidence for fibromyalgia from the GP and asked if I have a phone number which I gave her, She then asked me loads of questions about my B/f mental and physical conditions. She did not say she would be contacting the surgery on the correct number and at the end said she would write up the report and give it to her manager to see if she wanted any more questions asked. My fear is that they are using out-of-date information altogether from 2017 and not information from the review form I sent back in November 22. Don't know what to do as I'm sure there has been an almighty cockup and that my B/F may lose his PIP. Please advise. Thank you.

I'm sorry that you're having to worry about what evidence they are looking at. You can check with the DWP what information they forwarded to Capita, but at this late stage, I don't think there is anything more you can do before the decision is made.
