How to ask for PIP reconsideration after re-assesstment? (from 12 to 0 points)

My PIP reassessment was refused nearly 40 days ago.
I didn't receive the decision letter until today.(I called them many times and was told it will be ok if it arrives later than one month).

I was claiming PIP for multiple conditions for 7 years.I had 2 reviews and both past reviews were successful with 8 points daily activity and 4 points mobility.

however this time they gave me 0 points.

the decision letter I receive is called PIP 7013.

it just says based on the information they have they can't award me PIP ,showing descriptors and 0 points for each.
then it goes on and simply dismissing the problems I explained.without any explanation or reason.

eg : you said you have problems with this task,I decided you can do it.

my question is,how do I ask for reconsideration? as there is no reasoning or explanation why think I can do the activities that I can explain.

is this the correct letter they sent me for requesting reconsideration?

I actually posted a new thread with questions about mandatory reconsideration before this reply was published.
it's really overwhelming.

There was no difference in the way I described my problems.in fact I used the exact same form from past, since my conditions and their effects remained exactly the same.
and of course I used the guides from here to complete it.

the only difference is that my past 2 assessments were face to face (pre-covid) but this one was over phone.
it's clear this HP didn't want to accept them and DM simply accepted HPs opinion.

as an example HP referred to one of the medications as "first line low dose" but it is in fact the second highest dose of that medication.
or HP completely ignored my old MRI scan and the fact that such condition isn't reversible once you have it you have it.
in short when someone has an agenda they can interpret things the way they want.
