I moved your question - from where you posted it. We ask people not to pose a question on someone else's thread as it can get confusing.

A PA4 is the name of the PIP assessment report, which is written if someone has a face-to-face, telephone or video assessment. Claimants can request a copy of the report 48 hours after an assessment has taken place.
