Hi Mikey Yes, yes, yes, I would definitely include a copy of that letter. And when you fill out the form, I would actually refer to that letter at a relevant point - just to highlight the issue that you have a prognosis that you will not improve.
The form I think you are referring to was for PIP and it's under the heading for GP Notes. There isn't an equivalent one for ESA/UC

Thanks for your help. I've never claimed PIP so I wouldn' t have used the doctor's letter template from the PIP guide. A template of the letter (available for download) used to be among the ESA guidance but it seems to have been removed?

Sorry, I have never seen the letter you refer to and I've tried to search for it. There must be a reason why they removed it. In the guide it just says take the descriptors along to the doctor's if you want them to write a letter.

I have a copy of that letter but I don't know how to upload it onto here? I used the same completed letter (signed by my doctor) previously for ESA WCA.
Regarding my returned WCA form. I forwarded it to arrive by July 21 - sent recorded delivery. That was 7 weeks ago - and I still haven't heard anything.
