Hello Gordon,
I would like to ask If I aked for another claim form would I be sent one? as I have messed my original,and already asked for a second extention, I'm getting that stressed!!!!!! I did on rough paper first and still messed it up. Feel like i,m going mad with it, can't cope.Please help! I am a member have been for sometime.I suffered Cauda Equine syndrome which has damage nerves in my spine horrendous pain, immobility for unspecified amounts of time. incontinence basically it isthe actual action of movement, I am rubbish of explaining myself. I am not tech savy is there anywhere else on your site I can use to ease this pressure we are put under? just to prove our living problems. I'm sure none of us choose to have our health issues.

Please don't post your questions in others topics, it can easily lead to the wrong advice being given or taken.

All you can do is ask. If they don't allow you one then you could prepare your answers on separate sheets and attach them to the form you have, , just make sure that each page has your full name and NIN at the top and that they are numbered, we recommend "page X of Y" so that it is easy to see if pages are missing. Make sure you clearly mark on the form where your answers are and on the relevant page that you are answering question X.

Attach the pages to the back of the PIP2 using a staple or make a hole in the top left corner and use a treasury tag, string or ribbon to the form. The pages need to be easily detached so that they can be scanned by the DWP.
