| Main conditions that affect daily living: After a slip I broke 1 vertebrate and caused 3 bulging disc in lower back causing nerve damage and pain in back and legs so unable to wash/dry lower half of body put on socks etc. I had spinal fusion surgery which was unsuccessful due to nerve damage. Unable to walk without intense pain. I also have 3 bulging discs in neck and stenosis causing pain in arms and hands and difficulty gripping /holding things and problems with motor skills co-ordination. Shoulder impingement again restricting movement in my right arm further. After working from leaving school had to retire on ill heath grounds and DWP placed in support group due to my condition. Which has since caused me problems with anxiety and depression. |

| Tasks you struggle with on a day-to-day basis: Unable to wash/dry lower half of body or above shoulder height even trying to use aids. Unable to put on socks again even using aids due to lack of grip. Difficulty preparing and cooking food unable to stand to cook even using perching stool too painful. Often cut self whilst preparing food due to lack of motor skills and have also scalded myself. Also i have osteoarthritis. |

| Expected problems if you tried moving into work, with support from a work coach: Unfortunately due to my conditions I was forced to leave employment for the Civil Service as I was unable to continue in my role nor could they find anything that was suitable and which I would be able to carryout. Also when assessed by a DWP Specialist Physiotherapist I was placed into the support group for ESA as I would be unable to carryout a job role due to my conditions. |
