Tasks you struggle with on a day-to-day basis: Not preparing simple meal from fresh due to fatigue, energy levels. Use microwave to warm up pre-prepared snacks (2 points).

Managing therapy or monitoring a health condition: often forget medication / risk overdosing, do not hear CMG alarm, Poor appetite, fatigue (1 point).

Shower once a week, unable to step out of bath or shower without assistance (maximum of 3 points).

Reading and understanding signs, symbols and words: lots of eye strain reading, nystagmus and retinal challenges.

Communication / Engaging with other people face-to-face: hearing aid wearer, still unable to hear people most of the time (2 points).

Budgeting: need help to Fluctuating mobility, sight, hearing due to vestibular dysfunction.

Understandable for employers, I'm much slower, have poor communication and visual competency, diabetes and balance issues mean risk of falls.

Volunteer maximum 4 hours once a week - flexible, due to fluctuating condition, fatigue - different expectation for paid staff.
Anything else to tell us: Without support, life cannot be sustained - I'd be a much higher risk of burden on health & social care, given previous psychosis / suicidal issues.
