| Tasks you struggle with on a day-to-day basis: I struggle with hands, feet and cognitive abilities, non restorative sleep abs fatigue. My memory and learning new processes is seriously affected by brain fog, I forget to attend appointments and recently my financial affairs have suffered as I have forgotten accounts and to pay things. I cannot peel or chop things as cannot use knives or scissors safely, I cannot grip or carry things due to lack of grip and weakness in my hands, I’m in too much pain on a daily basis to sit or stand comfortably. Since my initial award I have developed back problems which is under investigation and the problem extends to my legs too. I have constant numbness and pins and needles in my back, legs, hands and feet. I suffer from osteoarthritis in my neck, hips, knees and feet making it very difficult to sit and walk, I am unbalanced in walking. Sometimes I cannot shower due to the pain I’m in. The pat 12 months has seen a decline in my mental health too. I have light and noise sensitivity and have to wear sunglasses in the house as well as outside and noise cancelling earphones to block out noise. This list is not exhaustive as everyday can bring new challenges and ailments. |

| Expected problems if you tried moving into work, with support from a work coach: I couldn’t work as much as I wanted to as I could not learn the processes required by any company due to my lack of focus and concentration and memory problems, I wound be taking time off when the pain was too great too. |
| Anything else to tell us: The worry of these proposals are seriously triggering my symptoms and affecting my mental health further. |
