| Tasks you struggle with on a day-to-day basis: I struggle to stand for any length if time unaided and often lose balance with aid. I can't walk without pain. I struggle with PEM and have to pace myself carefully to not cause a flare. This includes regular rest breaks through the day where I need to lie down and sleep. I can't cook or prepare meals. I use aids to eat and drink. I use kids to use the bathroom. Aids to shower. |

| Anything else to tell us: I use my PIP to help care for my basic living needs to allow me to work, if I lose this I fear I will not be able to care for my basic needs and therefore would not be able to continue in my job. I could become unemployed as well as unable to care fir my basic needs. |
