You will need to write to the DWP office which handles your ESA claim, and explain that your medical condition has now deteriorated, and you wish to be considered for the support group.
The DWP will carry out a supersession of your current ESA award which will in all probabilities lead to another work capability assessment, commencing with the issue of an ESA50 to assess the extent of your health deterioration.
Could that assessment end up with the person being found fit for work ?

In order to be found eligible for the ESA Support Group, you will need to demonstrate that you meet one or more of the relevant criteria, these are explained in the Understanding ESA guide in the Members area.
As CD has said, this will more than likely mean that you will receive a new ESA50 to complete, you need to be aware that the DWP will review your case in full, and whils it is rare, this can result in your losing your existing WRAG status, so think carefully before doing this and if possible consider getting face to face advice from your local CAB or Welfare Rights.

As Gordon says, there is unfortunately a caveat that asking for a supersession of a subsisting ESA award, could possibly result in a claimant being found fit for work.
