There is a free program for filling in non-interactive PDF forms which is available HERE.
I gave it a trial run on the first page of the existing ESA50, and despite the fact I found it a little bit fiddly at first, once I got the hang of it, it appeared to complete satisfactorily. I didn't print the page off though, so I am unaware of what the finished article will actually resemble?
