My wife received a letter from DWP in november regarding her DLA claim stating they have requested a report from her Physiotherapist.She has now received another in January stating they have requested a G.P.report.She has the full backing of her GP regarding her claim but will this report be in the form of a questionaire or a written report as her GP has requested her to visit the surgery and go through it with her.If it is a questionaire is there anywhere on site that shows it or will it just be an overall written report about her conditions.

depending whats wrong with her and who is asking the questions sets if its a set form or questions to answer something thats not clear

Hi Ginger,

there are a number of additional evidence forms which are sent to GPs depending on the claimant's condition, but I don't think we have copies on the site.

However, it's excellent news that your wife's GP wants to talk to her first - many GPs just fill the form in off the top of their heads without bothering to check with their patient. Indeed, the notes with the form tell the GP they don't need to talk to their patient but should just complete the form using their patients notes and their own knowledge of the patient.

Hi Ginger

As Steve has said it is good that your GP has asked your wife in to go through the form. My friend was in a similar situation 6 months ago, her consultant got a similar letter to complete and he requested my friend go in to see him in order to complete it. Her consultant was able to write down how her disability affected her but also had the oppertunity to add further information. This was in favour for my friends disability and her DLA was continued at the same rate.

I have a booklet with certain conditions and the questions to ask would you like a copy ? ;)
