SDP/EDP on ESA - will I be invited to apply?

Thanks to the wonderful advice from B&W, I was awarded EDL and EM in my PIP assessment last September, having been placed in the SG for ESA since May 2015, for twelve months. (My ESA decision notice does not state whether my award is Income-related or Contribution-based, but I would guess that as I was working until January 2015 that it was based on my NI contributions for the tax year 2013-2014.)
I have seen from other posts that some folk have received a form from the DWP to apply for Severe Disability Premium or Enhanced Disability Premium on their ESA awards. My question is this: do I wait for the DWP to invite me to apply for SDP/EDP on my SG ESA award or do I write and tell them the results of my PIP Assessment? On the other hand, as I was only placed in the SG until May this year, will they reassess me then and automatically consider the EDP/SDP Premiums?

Ask them if you on income or contribution based ESA as in same situation they told me mine is a combination of contribution based and income related.

To be entitled to payment of any of the : Income Related (IR) ESA Premiums you must fulfil the qualifying criteria as laid down in this link, and you must also be able to pass the IR ESA means test with respect to saving, capital, assets, household income and any hours worked by a partner if you have one.
Asset rule for receipt of IR ESA
24 hour work rule for payment of IR ESA
If you can't pass the IR ESA means test, you will not receive payment of any ESA Premiums even if you fulfil the other qualifying criteria.
Even if you are being paid CB ESA, you can still receive IR ESA Premiums top ups in addition to your CB ESA if you can pass the means test.
Even though your CB ESA payments will be counted as Household Income for the purposes of the IR ESA means test., your PIP payments will not be.
Being entitled to ESA Premiums increases the amount that the Government state that you need to live on.
You cannot always rely on The DWP to assess you for payment of IR ESA, so if you wish to be assessed financially to see whether you are entitled to payment of any IR ESA top-ups, you need to contact ESA :
THE ESA PHONE NUMBER DWP : 0345 608 8545 (Open 8AM-6PM. Mon-Fri.)
Request that they send you an ESA 3 Form which will be used to capture your personal and financial details.

There is no time limit on payment of arrears of any IR ESA that you may be owed !

Many thanks for your comprehensive answer - and sorry, I'd completely forgotten that I'd asked part of the question already; I'm afraid my memory isn't what it used to be!
I will write and ask the DWP for form ESA 3 as I'm not good on the phone and I like to try and keep track of everything on paper.
I really can't thank B&W enough for all the invaluable help and advice I continue to receive - communication between Government Departments seems to be negligible and "the left hand knoweth not what the right hand doeth" so having you here to guide me is an absolute "must". I shall certainly be renewing my subscription!
