I have a few questions. They say i'm entitled to contribution based ESA;this I do not understand. More importantly no re-assessment time is stated in my letter , the prognosis.Also on the final page of my letter I am informed that "The amounts on this page apply from 15th October 2013 to 25 November 2013 ". To me this is meaningless. I would be grateful for your assistance !.

Congratulations on The SG award !!
Thank you for the positive feed back.
Whether or not you will be paid CB ESA will be dependent on your NI Contribution history gained from employment before claiming ESA, and/or which benefit that you have been transferred over from, to ESA, if that is the case. :
What will I be transferred to?

The good news is, that there is no 385 days limit to payment of CB ESA nor IR ESA if you are in The SG, for as long as you remain in The SG.
To find out the recommended period of time before you are reassessed,(The Prognosis) you can ring the telephone number on the SG award notice letter, they should have it on-screen.
You can also request the ESA85 medical report in writing, which should have the prognosis and the reasons why you were placed into The SG therein. :
ESA medical – what forms to ask for?
SG awards generally attract a longer prognosis, the maximum being 3 years. :

As you will see I have already answered some of your queries, and have merged your posts.
With regards to this query :

The decision notice letter does not include the prognosis.
The November date that you mention is purely for administration purposes, and is tied in with payment of The Xmas Bonus, which is payable from the next pay week after the November date.
So this does not mean that your ESA will cease on 25/11/13.
The DWP should be able to inform you whether you are being paid CB ESA, IR ESA or both, if the information that I have already provided dos not answer your question regarding this.
