This is my story to give some hope to all you worried people - I have severe OCD,Depression,Anxiety andsome physical problems:-
Oct 2009 Ib Review due and heard nothing
July 23 2011 Migration letter rec'd
July 28 Dwp follow up call - did not answer phone
Aug 5 Rec'd ESA 50
Sept 8 Posted Esa 50 Recorded Delivery to Atos & copy to DM, plus 12 sides of A4 of very detailed info on how my condition impacts my everyday life & brief support letter from old GP ( who has since left the practice)and print out of Meds.
Sept 22 Contacted by new Gp who had rec'd ESA113
Oct 18 Letter from DWP & in SUPPORT GROUP!!
The relief is just amazing - I feel as though I'm floating on a big fluffy cloud! I had feared the worst as I do 3 to 4 hours Permitted Work each week but I made sure I told them in detail about it and that it didn't conflict with descriptors I hoped to score on.
Huge thanks to Steve and all hard working Mods to keep this fantastic site going. All I need to do now is to write to thwe DM to find out my next review date and how I managed to get in the SG - What form do I ask for? - then I can relax until next time.
Good Luck to all of you - it has been two years of sheer hell for me waiting for it to happen - but if I can do it with the help of this site - be encouraged that you could do it too!
Much love and grateful thanks

Hi Fran,
Really pleased to hear of your excellent news.
Thanks for your comments about B & W, and for sharing with us and the members.
The forms that you should request are an ESA85a, and a Written Staement of Reasons, ESA65.
It may be wise to mention these forms in particular, and also request any information that they used to come to their decision, this would include the ESA113 that your G.P. filled in.
Obviously you could inform them that you don't need copy of anything that you provided, if you kept copies yourself.
Well done again.

Delighted for you on a good result.
My review is due soon and I wonder if you could tell me the address of the decision maker, or where to find it,as I want to send a copy of ESA150 to him as well as Atos.
Thankyou in advance and best wishes.
