My wife recently had her renewal forms for DLA. We filled these in with a lot of help from B&W and returned them 3 weeks ago.
I scanned the forms to my pc with the intention to printing them off. Unfortunately i got a virus on my pc which wiped my entire hard drive before the forms were printed. we now have no copy at all of any of the form.
My question is, would the DWP send us a copy of the forms, if I asked? If the worst happens and she gets refused DLA, without any copy to fall back on, she would be never have a chance of winning any appeal.
You can ask although I doubt they are under any obligation to help you, perhaps if you offered to cover any copying costs, 10p a sheet should be reasonable.
With regard to an appeal, the application form should be part of the evidence pack that the DWP put together for the Tribunal Service.
