DLA renewal or new claim

I have just received the DLA letter and claim form DLA1A Adult May 2010.
In the cover letter they say "your award is due to end /08/2011. But you can claim DLA again. We are sending you a form to claim again with this letter.

I was Awarded LR Mob & Care August 2009 - August 2011 2 years.

They sent a new claim form. My condition is a little worse. I will claim again.

Q. Should I have been sent a renewal form and not a new claim form?

I was under the impression I was getting the new 4 page Con trick renewal form.

They do send a new claim form rather than the four page form to some people. You don't need to worry about it being a mistake.

Why to some only and what is the purpose? Which is better for me New claim forms or 4 page renewal?

I'm afraid I don't know why.

A new claim form gives you an opportunity to include more information, although you can append extra pages to either. Either way, beware tick boxes and if you don't have space to give an adequate answer, add more at the end.

If I ever got the smaller form I would just print the big one off the web and use that, I dont think the small one is a good idea

This is exactly what I did. A 4 page form is of no use if you have a complicated illness or disability. I printed off the proper form, my helper filled in both of the forms and cross referenced them both and in some parts of the form wrote 'not enough space please see....' . It will take a while to do but please fill in the form throughly, photocopy it and send it registered post to them (you can phone the postal delivery service the next day to see if it has arrived).
