Today I have recieved a DLA-580 Entitled Disability Living Allowance- Claiming again for persons over 16. I am on High rate care and Mobility Component at the moment to be reviewed in June. This form is a total of 8 pages. Very basic? Has any one here had any experience of this form and should I be worried.
