A while ago I saw that there is a form you can take to your GP to ensure that they are properly up to date with your condition and give an accurate report when the DWP ask them about your conditions for DLA claim. Does anyone know where to get it as I can't find it ANYWHERE! Thanks, Jen
I believe the forms to which you refer are on Page 61 of the guide entitled "The Best Possible Disability Living Allowance Claims For Adults On Physical Health Grounds", and page 53 0f the guide entitled "The Best Possible Disability Living Allowance Claims For Adults On Mental Health Grounds".
Both it would appear are the same form despite being in separate guides, and can be found in the DLA resources area HERE.
