Hello,I'm new to this forum and hope someone can give me some advice.For the last 4 years I've been on Income support,due to Ill health,Depression,Anxiety and Stress.I had my last medical with Atos 2 years ago and passed,but was not moved to ESA.This morning I received this Income support review claim form,which I've never had before.Its mainly about what money I've got coming in,if I own any property and if I live with anyone.There's a little box at the end of the form asking if I claim because I'm sick,which Iam,and just to list whats wrong with me.There's nothing about who my GP is or what medication I'm taking.I would appreciate if anyone could tell me what this form is about.Thank you very much.

All government forms should have a form number printed somewhere on the page, usually at the bottom, does yours?

Income Support is means-tested, so the information about what money you've got coming in and whether you own any property is relevant to how much they pay you.

If you're also on DLA, whether you live with someone may be relevant to your rate of IS, depending on your rate of DLA.

If you take a look in the Help for Claimants area, you will find information in the incapacity benefit stuff about exemptions from PCAs and you may want to consider whether you may be exempt. If you think that you are, it would be worth mentioning (on an extra piece of paper if necessary) information that would help them also to reach that conclusion.
