There used to be a serperate medical that would be conducted after the WCA but that was stopped sometime ago, it is now usually incorporated into the WCA, however, there has to be a reasonable expectation that you would meet the requirements, in order for the test to be carried out.
Unfortunately it is perfectly possible to suffer from constant pain and difficulties with standing, sitting and walking, but still not be eligible for the Support Group. It is important that you understand that the criteria for the Support Group are specific and need to be supported by appropriate evidence.
As an example, if you are unable to walk 50 metres, you would be eligible for the WRAG, however, if you can show that you cannot walk 30 metres, you would eligible for the Support Group.

Silly question? is this a special person in CAB who works for cab? si its them I call? as I have never spoke to anyone before.

There are many different names for people who give benefit advice, Welfare Rights Officers/Advisers, Benefit Advisers. They all mean the same thing. Your local CAB should have such advisers (their called Welfare Benefit Caseworkers) who can assist you.
From the letter you mentioned it sounds like the DWP are unclear whether you want the decision reconsidered or are just making a complaint. It's important to state in writing that you want the decision reconsidered and give reasons, but I think it would be best if you show the CAB your letters and let them start the process.
