IS there a copy of an ib50 on this site?

Hello sorry if this is a daft question and i have looked but is there a copy of an IB50 on this site, have looked but coul not find one. Maybe i am not looking in right place. Need to check something out and have not got a copy of mine anymore.

I can't find an IB50, but if you look at the guides to claims for IB, you'll find they take you filling in an IB50.

I have a hard copy so if you PM me I can look up whatever you want or if I can email you a scanned copy of the whole form (plus a lot of my scibblings on it!!!) to your main email address

I would really appreciate a copy of the old style IB50 as i have binned mine from my last PCA which was last year, or rather my other half did.
I am doing some "looking" at the wording.
