DLA Change of Circumstances: lost orig claim pack

Help please! How could I have been so stupid? I've just applied to Blackpool for DLA Change of Circumstances forms as my health problems have got worse, and find I have lost my copy of my original claim pack and award notice (for medium care and low mobility). All I have is the annual confirmation letter showing my current entitlement is "from 12.01.1994 indefinitely".

Any ideas pls as to how I can get hold of a copy of my original claim pack? It would help so much to truthfully beef up my new application.

You can ask the DWP for a copy of your original claim form. I don't know how long it takes to get it.

I've always written to Warbreck House when I contact the DLA people, although it's anybody's guess where they actually store the files.
