Is it possible to get a copy of my original DLA claim form from some 18 or so years ago under the freedom of information laws?

Last year I managed to get a two-year-old copy of my IB50 medical form just by calling the Benefit Delivery Centre. It's worth giving them a ring as they should still have it in storage.

Hi,
It's highly unlikely that the DWP will have a copy of an original claim form if it was made 18 years ago, it could have been destroyed under the provisions of the Data Protection Act 1998.
You need to contact the DLA Unit at Warbreck House in Blackpool on 08457 123456 and ask if they have a copy, if so, it's sufficient just to ask them for a copy, although this is sometimes called a Subject Access Request.
It's not necessary to use the Freedom of Information Act, that is used to seek information about a government department rather than an individual.

Hi, I live in the North West and was told by a manager at the JCP that they store records going back years at a place in Preston! I think under the data protection it states 6 years. So like jim1 said i would contact the DLA. By the way jim how is pat? hope all is well, Best Wishes
