ESA equivalent of the Medical Visit Record Sheet?

In addition to my second assessment for ESA, I'm also doing a supercession on DLA because of my deteriorating condition, and I saw the Medical Visit Record Sheet.

It looks very helpful for helping to remember what occurred. I'm going to try to take notes at my ESA medical (last time, they said I wasn't allowed. ). But if they tell me no again, I will certainly want to write down what happened as soon as I can.

Is there a similar questionnaire for the ESA medical? If not I'll just try and adapt the DLA one.

I must admit I cannot find one in the members area. I know Steve has been/is tied up a lot at the moment, but perhaps that is one to do in the future.

There isn't a similar sheet for the ESA medical. But it's an interesting idea which we will look into for the next update.
